Personal Injury Policy


When an individual ( employee, student or guest) is injured ( or his personal prope11y is damaged) while on the College's prope11y or while off-campus but on official College business, he/she should do the following:

For JSCC Employees

The HR Director or Environmental, Health and Safety Training Coordinator (EHST) must be contacted prior to the employee receiving medical treatment, unless it's an emergency situation.

Call or have someone call an ambulance if needed.

There are several people on campus trained in First Aid and CPR. If an injury has occurred that needs first aid, please make sure the injured employee is aided by one of our trained employees.

Workers compensation paperwork for JSCC employees should be filled out if any of the following has occurred:

  • Deep cut that will not stop bleeding
  • Cut that could become infected
  • Complaints of wrist pain
  • Complaints of back pain
  • Possible broken bones
  • Sprains
  • Falls
  • Major accidents
  • Complaints of any muscle pains

When an injury is reported, the "Supervisor's First Report of Incident" is the first form that needs to be completed. This form can be obtained from the Human Resources office. Most of the information on this form can be gathered from the injured employee. The employee then completes the "Employee's First Report of Injury" form. This form can also be obtained from Human Resources. The employee should fill out this form prior to seeing a doctor. The only reason why this should not happen is if the employee is bleeding heavily or if the employee is taken to the emergency room. The employee then completes the Employee Medical Selection Report which is, by law, a listing of three (3) doctors from which to choose from to care for injuries received as a result of the accident. The Medical Provider should complete the "Physician's Treatment and Return to Work" form and return it to the employee before they leave the office. If the employee can't return to work immediately following treatment, the physician should fax this form to the HR department.

For JSCC Students and Guests

Contact Campus Security as soon as possible. Once on the scene, security will assess the situation and make a report from victim and witness statements. The report will be shared with the Dean of Student immediately for any additional follow- ups to be done with the victim and/or to make class arrangements.

The victim of the accident should have all medical bills including ambulance services charged to themselves or their Insurance Provider. The victim should then contact JSCC for a copy of the incident report to attach to the "Claim for Damages" form obtained from the Vice President for Financial & Administrative Affairs Office or www.tdot.state.tn.us/legal/claimfom1.pdf. The information should be mailed directly to:
