What changes can I make?

Transfer medical plans (Blue Cross Blue Shield and Cigna, Partnership or Standard Plan)
HOW? Complete the Enrollment/Change Application and forward to Human Resources.

Enroll, transfer, or cancel dental plans (Assurant Prepaid and Delta Preferred)
HOW? Complete the Enrollment/Change Application and forward to Human Resources.

Enroll in optional special accident
HOW? Complete the Optional Special Accident Enrollment Form and forward to Human Resources.

Cancel optional special accident
HOW? Complete Optional Special Accident Enrollment Form, marking “I wish to terminate coverage” and forward to Human Resources.

Enroll in optional term life coverage
HOW? Complete the Minnesota Life Application Form, marking “Annual Enrollment,” complete the Supplemental Application Form and forward to Human Resources.  OR Enroll online at https://web1.lifebenefits.com/lbwcm/pd/tennessee

Increase optional term or universal life coverage
HOW? Unum Provident or Minnesota Life will mail forms to employees eligible for the $5,000 guaranteed issue. Complete the increase form and forward to Human Resources. If you believe you were eligible for an increase but did not receive a letter from Unum or Minnesota Life, please call customer service at 1.800.635.5597.

Cancel optional term or universal life coverage
HOW? Complete the Customer Service Request Form and forward to Human Resources.

Enroll or re-enroll in Flexible Spending Account Program
HOW? Complete the FBMC Enrollment Form and forward to Human Resources.
